Ethiopia and the United States have signed a historic $1.6 billion health cooperation agreement, marking one of the most significant health sector partnerships between the two nations and signaling a shift toward more direct and sustainable development cooperation.

Under the agreement, the U.S. government will contribute $1.16 billion, while the Ethiopian government will cover the remaining share to strengthen domestic health financing. The initiative will be implemented over five years and is designed to bolster Ethiopia’s health system, enhance service delivery, improve access to quality care, and ensure long-term institutional resilience.

The agreement was signed in Addis Ababa by Finance Minister Ahmed Shide, Minister of Health Dr. Mekdes Daba, and U.S. Ambassador to Ethiopia Ervin Masinga. Both governments described the initiative as a milestone in Ethiopia–U.S. relations and a defining step toward deeper, more accountable, and results-oriented cooperation.

A Shift From NGO Reliance to Direct Partnership

Beyond the financial scale, the agreement is being viewed as transformational because it represents a move away from traditional NGO-driven aid practices that often offered limited long-term impact, fragmented implementation, and weak accountability mechanisms.

Instead, this new approach prioritizes government-to-government partnership, shared responsibility, and sustainable capacity-building. It reinforces Ethiopia’s leadership in managing its health priorities while ensuring that U.S. support directly strengthens national systems rather than operating parallel to them.

Reflecting Evolving U.S. Aid Policy Toward Africa

The agreement also aligns with evolving U.S. aid principles under the Trump Administration, which emphasized reevaluating large intermediary aid structures and encouraging more direct, strategic partnerships with national governments. While the administration proposed shifts in how funding is channeled—including reforms affecting major institutions like USAID—the broader objective has focused on ensuring that assistance leads to measurable, mutually beneficial outcomes and stronger state institutions rather than long-term aid dependency.

Strengthening a 120-Year Partnership

Officials emphasized that this agreement builds on over 120 years of diplomatic friendship between Ethiopia and the United States, marked by cooperation in health, development, education, security, and humanitarian response.

Described as a “new chapter” in bilateral cooperation, the initiative is expected to contribute significantly to Ethiopia’s health development goals while reinforcing a model of collaborative investment, shared accountability, and sustainable progress.
